ORC completed its Q3 2024 safety review on 14 October 2024. Zero lost-time incidents were recorded. This is the fourteenth consecutive quarter in which ORC has recorded zero lost-time incidents.

A safety certificate has been ordered to mark this milestone. The supplier has been chased twice. The estimated delivery date is unknown. The previous certificate is displayed in reception. The previous certificate expired in Q2 2021. The gap between certificates is acknowledged. L. Sandhu has noted the gap.

Three near-miss events were recorded during Q3. All three were reviewed by the Safety Committee. None met the threshold for formal regulatory notification under the UK Space Industry Act 2018. Two were straightforwardly below threshold. The third required a brief discussion about what “straightforwardly” means in this context. The conclusion was that it was below threshold. The discussion has been minuted.

Proximity alert triggered 6 August 2024, 14:22 UTC, during Patience routine transit at LEO 430 km. Automated collision avoidance manoeuvre executed per standard protocol. Delta-V cost: 4.2 m/s. The proximate object was a catalogued debris fragment (NORAD ID: 47291). Conjunction probability: 1:3,400 at closest approach. Avoidance at this probability is standard practice rather than a reportable event. No crew or mission impact. Logged and closed.

Mission plan submitted without Stage 3 approval signature. Identified pre-launch by L. Sandhu.
BELOW THRESHOLD
On 19 September 2024, the mission plan for ORC-2024-017 was submitted without the Stage 3 authorisation signature from L. Sandhu. The omission was identified by L. Sandhu during the pre-launch documentation check, which L. Sandhu conducts personally and has conducted personally since 2018 specifically because of events in 2018 that are not described in this report. The plan was returned, signed, and re-submitted. Launch proceeded on schedule. L. Sandhu has updated the pre-submission checklist. The new field is mandatory.

Anomalous docking arm reading during approach on ORC-2024-015. Sequence paused. Under review.

During the retrieval approach for ORC-2024-015, the docking arm registered a reading that S. Davies described in his incident note as “inconsistent with the object profile and inconsistent with anything I have seen before in eleven years.” The retrieval sequence was paused. S. Davies withdrew to a safe distance per protocol. The object was observed for 2.4 orbital periods. The anomalous reading did not recur. Retrieval completed successfully on the second approach.

  • The signal monitoring protocol for all active missions has been updated. All uplink frequencies are now continuously logged with automatic flagging for any signal that does not match a registered format. This was sensible practice regardless of Q3 events. It should have been done earlier.
  • The pre-submission checklist has been updated to include a mandatory Stage 3 signature field. This has been confirmed by L. Sandhu. L. Sandhu has asked that the update not be described as “overdue.” The update is timely.
  • NM-2024-Q3-03 remains under internal review by M. Hargreaves. A target completion date has not been set. M. Hargreaves is aware that a target completion date has not been set.
  • All near-miss events should be reported within 24 hours to L. Sandhu using Form ORC-SAFETY-007(b). Not ORC-SAFETY-007. ORC-SAFETY-007(b). The difference is documented in the guidance notes.
✓ CANTEEN MICROWAVE SITUATION — RESOLVED The canteen microwave situation, raised as an outstanding facilities matter in the Q2 2024 safety review, has been resolved. The resolution is considered satisfactory by most parties. The matter is closed. It will not be referenced in future safety reviews unless circumstances change. L. Sandhu has confirmed this.
