On 14 August 2024, ORC received a retrieval request submitted on Form ORC-088. Form ORC-088 was superseded by Form ORC-089(b) in March 2021. The supersession was communicated to all registered clients via the client portal, the March 2021 newsletter, and a direct email sent twice.
The form was processed in good faith until the mission planning stage, when P. Patel noted that the object described in section 4 did not correspond to the object at the stated orbital coordinates. The coordinates were correct. The object description was not. This distinction is significant. The significance is documented in the supplementary log.
T. Davies was involved in the subsequent discussion with the client. The nature of T. Davies’s involvement is documented in the supplementary log (ORC-INC-2024-008-SUPP). Access is restricted to N. Osei and above. The matter has been resolved. All parties have agreed to move forward. No further elaboration is available.
Form ORC-088 is no longer available for download from the ORC client portal. For reference, the superseded form is reproduced below so that clients with a saved copy can confirm it should not be used.
